What Medicare costs: every number, and which ones you can change
Short answer: a typical person on Original Medicare pays $202.90 a month for Part B in 2026, plus roughly $100 to $300 for a Medigap policy and $0 to $60 for a drug plan, with Part A usually free. On Medicare Advantage, many plans add $0 to the Part B premium but charge copays as you go, capped by an annual maximum. Higher earners pay a surcharge based on a tax return from two years ago. Here is every figure that applies, what is confirmed for 2027, what is still a projection, and the four programs that reduce these costs for people who qualify.
Key takeaways
- 2026: Part B is $202.90 a month with a $283 deductible; Part A is usually $0 with a $1,736 deductible per benefit period.
- The 2027 Part B premium was still unannounced as of October 12, 2026; Trustees project about $209.50, which is not official.
- Confirmed for 2027: the Part D out-of-pocket cap rises to $2,400 and the deductible ceiling to $700.
- IRMAA surcharges start above $109,000 single or $218,000 joint, based on your 2024 tax return for 2026 premiums.
- Medicare Savings Programs, Extra Help, and the Part D cap can cut these costs substantially for people who qualify.
Part B: the premium everyone pays
| Item | 2026 | 2027 |
|---|---|---|
| Standard monthly premium | $202.90 | Not yet announced; Trustees project about $209.50 |
| Annual deductible | $283 | Projected near $292 |
| Coinsurance after the deductible | 20%, with no annual cap | Same structure |
| Immunosuppressive drug premium (Part B-ID) | $121.60 | Not yet announced |
The 2027 Part B figures were still unpublished on October 12, 2026, three days before open enrollment opened. CMS normally announces in November. Projections come from the 2026 Medicare Trustees Report and are not official amounts.
Part B is deducted from your Social Security payment if you receive one, which is why a premium increase shows up as a smaller deposit rather than a bill (why your check shrank). A protection called hold harmless generally prevents the standard premium increase from exceeding your cost-of-living adjustment for most people already receiving benefits, though it does not protect people paying IRMAA.
Part A: free for most, expensive if not
| Item | 2026 amount |
|---|---|
| Premium with 40+ quarters of covered work | $0 |
| Premium with 30 to 39 quarters | $311 a month |
| Premium with fewer than 30 quarters | $565 a month |
| Hospital deductible | $1,736 per benefit period |
| Hospital coinsurance, days 61 to 90 | $434 a day |
| Lifetime reserve days (60 total, ever) | $868 a day |
| Skilled nursing, days 21 to 100 | $217 a day |
The deductible resets per benefit period, not per calendar year. A benefit period ends after 60 consecutive days without inpatient or skilled care.
That per-benefit-period rule is the most misunderstood cost in Medicare and is explained fully in the hospital guide. Buying into Part A without enough work history is covered in the eligibility article, and the spouse-record route that often makes it free is in the spouse article.
Part D: drug costs, with a real ceiling
| Item | 2026 | 2027 |
|---|---|---|
| Maximum deductible | $615 | $700 (final) |
| Out-of-pocket cap on covered drugs | $2,100 | $2,400 (final) |
| National base beneficiary premium | $38.99 | $41.33 (final) |
| Average standalone plan premium | $35.09 | About $36 (projected by CMS) |
| Insulin, per month’s supply | $35 | $35 |
| Recommended vaccines | $0 | $0 |
Part D figures for 2027 are final, confirmed in CMS announcements. Once your out-of-pocket spending hits the cap, covered drugs cost nothing for the rest of the year, and every plan must offer monthly payment smoothing.
The cap is the most valuable change in Medicare in a generation for people with expensive prescriptions. The Part D guide covers the phases, and the cap article explains smoothing.
Medigap and Advantage premiums
Medigap premiums are not standardized even though the benefits are: the same Plan G can differ two or three times over between insurers in one ZIP code, and the pricing method (attained-age, issue-age, or community-rated) determines whether the policy that is cheap at 65 becomes expensive at 80. The pricing article explains the trap. Medicare Advantage plans often charge $0 beyond the Part B premium; CMS projects the weighted average Advantage premium falling to about $12 a month in 2027. What you pay instead is copays, bounded by the plan’s out-of-pocket maximum, commonly several thousand dollars in-network.
Income surcharges (IRMAA)
| 2024 income: single / joint | 2026 Part B total monthly | Part D surcharge |
|---|---|---|
| $109,000 or less / $218,000 or less | $202.90 | None |
| Above $109,000 to $137,000 / $218,000 to $274,000 | $284.10 | +$14.50 |
| Above $137,000 to $171,000 / $274,000 to $342,000 | $405.80 | +$37.50 |
| Above $171,000 to $205,000 / $342,000 to $410,000 | $527.50 | +$60.40 |
| Above $205,000 to below $500,000 / $410,000 to below $750,000 | $649.20 | rising toward the top tier |
| $500,000 or more / $750,000 or more | $689.90 | +$91.00 |
Based on your tax return from two years earlier. If a life-changing event has reduced your income since, file form SSA-44 to have current income used instead.
IRMAA catches people the year after they sell a house, do a Roth conversion, or retire. All of those are appealable life events or planning problems; the IRMAA article covers both.
The four ways to lower the bill
- Medicare Savings Programs pay the Part B premium, and in the QMB tier also deductibles and coinsurance. Income limits are higher than most people assume.
- Extra Help cuts drug costs to a few dollars and often eliminates the Part D premium.
- Re-shopping your drug plan every fall. The spread between plans on the same prescription list is routinely hundreds of dollars a year (how to compare).
- Appealing IRMAA with form SSA-44 after retirement, widowhood, divorce, or loss of income-producing property.
